Abstract
Purpose
The clinical, MRI and EEG findings of surgically treated patients with partial epilepsies due to NMD with those of patients with BTU and HS were compared.
Methods
The findings of 41 consecutive patients with NMD were compared with those of 20 BTU and 17 HS patients.
Results
A predominantly or exclusively temporal localization of the brain abnormalities on MRI (85, 100%), with interictal (95, 100%) and ictal EEG (85, 88.2%) was found in BTU and HS patients, and a more frequently extratemporal localization (58.1, 58, 68.6%) in those with NMD. Retarded developmental milestones (46.3%), intellectual deficits (78.4%), behavioral disorders (68.3%), neurological deficits (61%) and a more unfavorable surgical outcome (class I A-D: 37.5%) were found more frequently in NMD patients, more favorable findings in those with BTU (0%, 45%, 65%, 10%, class I A–D: 80%) and those with HS (6.3, 29.4, 29.4, 29.4%, class I–D: 64.7%). Possible causal factors for these differences are discussed. Etiologically only the occurrence of preceded febrile seizures (Fs) in HS patients can be considered as essential causal events.
Conclusion
Partial seizures due to NMD are on average unfavorable seizure types in comparison with those due to BTU and HS.

Zielsetzung
Es sollten klinische, MRT- und EEG-Befunde von chirurgisch behandelten Patienten mit NMS mit denen von Patienten mit HTU und HS verglichen werden.
Methode
Die Befunde von 41 konsekutiven Patienten mit NMS wurden mit denen von 20 HTU- und 17 HS-Patienten verglichen.
Ergebnisse
Bei Patienten mit HTU und HS war mittels MRT (85, 100%) und interiktalen (95, 100%) sowie iktalen EEGs (85, 88,2%) eine vorwiegende oder ausschließliche temporale Lokalisation der Abnormität, bei solchen mit NMS eine meist extratemporale (58,1%, 58%, 68,6%) zu finden. Eine verzögerte frühkindliche Entwicklung (46,3%), Intelligenzminderung (78,4%), Verhaltensstörungen (68,3%), pathologische neurologische Befunde (61%) und mäßige Operationsergebnisse (Klasse I A–D: 37,5%) waren bei Patienten mit NMS, wesentlich günstigere Befunde bei denen mit HTU (0%, 45%, 65%, 10%, Klasse I A–D: 80%) und denen mit HS (6,3, 29,4, 29,4, 29,4%, Klasse I A–D: 64,7%) zu finden. Mögliche Ursachen für diese unterschiedlichen Befunde werden diskutiert. Ätiologisch konnten lediglich vorausgegangene Fieberkrämpfe (Fk) (47,1%) bei den HS-Patienten als wesentliche Ursache angesehen werden.
Schlussfolgerung
Partialanfälle infolge NMS sind durchschnittlich ungünstigere Anfallstypen im Vergleich zu denen mit HTU und HS.
