Effizienz eines Monotherapiewechsels in der Behandlung von Epilepsien

Summary

If a first anticonvulsant monotherapy fails, a switch to another anticonvulsant drug may be an effective therapeutic regimen. We retrospectively analyzed the efficacy of a switch of monotherapy in 41 patients. In most patients medication was changed because the previous monotherapy failed to control seizures completely. A decrease of seizure frequency by at least 50% was observed in 53% of the monotherapy changes.We conclude that a switch to a another monotherapy is an effective treatment option before anticonvulsant comedication is established.

Zusammenfassung

Der Wechsel einer unzureichend wirksamen antikonvulsiven Monotherapie zu einer alternativen Monotherapie kann erfolgreich sein. 48 Therapiewechsel bei 41 Patienten wurden analysiert. Hauptsächlich erfolgte der Behandlungswechsel wegen Ineffizienz. Eine mehr als 50%ige Minderung der Anfallsfrequenz wurde bei 53% der auswertbaren 36 Monotherapiewechsel erzielt. Der Monotherapiewechsel ist somit, auch im Kontext früher publizierter Untersuchungsergebnisse, eine erfolgversprechende Option. Dennoch sollte ein Monotherapiewechsel nicht unlimitiert perpetuiert werden, da der Erfolg einer späteren Komedikation dadurch möglicherweise geringer ausfällt.
