Hormonelle Kinderwunschbehandlung und Verbotsliste der World Anti-Doping Agency (WADA)

Zusammenfassung

Viele Wirkstoffe, die zur hormonellen Kinderwunschbehandlung eingesetzt werden, sind Bestandteil der im Sport geltenden sog. Verbotsliste. Bei Frauen werden zur Stimulation der Ovulation GnRH(„gonadotropin-releasing hormone“)-Rezeptor-Agonisten oder -Antagonisten, Gonadotropine, Antiestrogene und/oder Aromatasehemmer eingesetzt. Nach der Follikelpunktion und bis in die Schwangerschaft hinein unterstützen Gestagene und humanes Choriongonadotropin die Aufrechterhaltung der Schwangerschaft. Liegen die Ursachen für eine Infertilität beim Mann, können – im „off-label use“ eingesetzte – Aromatasehemmer und Antiestrogene die Spermienproduktion und -beweglichkeit verbessern. Gleichwohl darf Leistungssportler*innen die Möglichkeit einer assistierten Reproduktionsbehandlung nicht per se vorenthalten werden. Dieser Beitrag erläutert die aktuell geltenden Anti-Doping-Bestimmungen, das Genehmigungsverfahren für die Anwendung verbotener Wirkstoffe bei Sportler*innen und enthält Tipps, wie behandelnde Ärzt*innen hier unterstützen können, sodass eine hormonelle Kinderwunschbehandlung im Einklang mit den Anti-Doping-Regeln durchgeführt werden kann.

Abstract

Many active ingredients used for hormonal fertility treatment are part of the so-called Prohibited List that applies in sports. In women, gonadotropin-releasing hormone (GnRH) receptor agonists or antagonists, gonadotropins, antiestrogens and/or aromatase inhibitors are used to stimulate ovulation. After the follicle puncture and into pregnancy, gestagens and human chorionic gonadotropin support the maintenance of the pregnancy. In cases of male infertility, aromatase inhibitors and antiestrogens used off-label can improve sperm production and mobility. Of course, competitive athletes should not be denied the possibility of assisted reproductive treatment per se. This article explains the current anti-doping regulations, the process for approval of the use of prohibited substances by athletes and contains tips on how treating physicians can provide support, so that hormonal fertility treatment can be carried out in accordance with the anti-doping rules.
