Zusammenfassung
Bei ungeschütztem Geschlechtsverkehr kommt es bei 8 von 100 Frauen zu einer Schwangerschaft. Methoden der postkoitalen Kontrazeption zur nachträglichen Verhinderung einer Schwangerschaft nach bereits erfolgtem ungeschütztem oder nicht effektiv geschütztem Geschlechtsverkehr verringern die Schwangerschaftsrate auf <2%. Aktuell in Deutschland als Notfallkontrazeption verfügbar sind 1,5 mg Levonorgestrel in einem Intervall von bis zu 72 h und 3 mg Ulipristalacetat in einem erweiterten Zeitfenster von bis zu 120 h nach ungeschütztem Geschlechtsverkehr. Beide Präparate sind verschreibungspflichtig.
Im „off-label use“ kann nach ungeschütztem Geschlechtsverkehr die Einlage eines kupferhaltigen Intrauterinpessars zur Verhinderung der Konzeption erfolgen. Vorteilhaft ist, dass im Anschluss eine längerfristige Kontrazeption besteht.
Abstract
Unprotected or ineffectively protected sexual intercourse results in pregnancy in approximately 8 out of 100 women. Methods of postcoital contraception for subsequent avoidance of pregnancy reduce the conception rate to less than 2 out of 100. Modern currently available methods of emergency postcoital contraception in Germany are single dose 1.5 mg levonorgestrel up to 72 h and 3 mg ulipristal acetate in an extended interval up to 120 h after unprotected sexual intercourse. Both medications are only available by prescription. The insertion of a copper-containing intrauterine pessary is an alternative method to prevent pregnancy in off-label use, with the benefit of ongoing, long-term contraception.
