Summary
The development of symptomatic hallux valgus deformities during childhood and adolescence is rather uncommon. Sometimes an operative treatment at an early age is indicated. The open growth plate at the basic phalanx and the metatarsal I basic have to be taken into consideration by the choice of operative procedure and osteosynthesis. A retrospective review of 15 minimally invasive modified Kramer osteotomies is presented with analysis of objective radiographic and subjective patient questionnaire results. The metatarsophalangeal angle (MPA) averaged 32° preoperatively and 13° postoperatively. The respective values of the intermetatarsal angle (IMA) were 12° and 5°. The position of the sesamoids was realigned to the first metatatarsal head. We conclude that the minimally invasive modified Kramer procedure is effective in correcting even severe Hallux valgus deformity without destroying the growth plate. The osteotomy is easy to control and the small incision saves the soft tissue. It is very important to take the distal metatarsal angle (DMA) into consideration by correcting the hallux valgus to provide a relapse.
Zusammenfassung
Die Entwicklung eines schmerzhaften Hallux valgus vor Abschluss der Adoleszenz ist eine eher seltene Erkrankung, die schon im Kindesalter eine operative Therapie erfordern kann. Vor allem das Vorhandensein von offenen Wachstumsfugen an der Basis der Grundphalanx und des Metatarsale I müssen bei der Auswahl von Operationstechnik und Osteosyntheseverfahren unbedingt berücksichtigt werden. Bei der hier vorgestellten Untersuchung werden Behandlungsergebnisse des Hallux valgus durch die minimalinvasiv modifizierte Methode nach Kramer in 15 Fällen demonstriert. Die röntgenmorphologischen Messungen ergaben eine durchschnittliche Korrektur des Metatarsophalangealwinkels (MPW) von 32° nach 13° und eine durchschnittliche Korrektur des Intermetatarsalwinkels (IMW) von 12° nach 5°. Die hier beschriebene Behandlungsmethode des Hallux valgus ist ein weichteilschonender, gut kontrollierbarer Eingriff, der auch bei großen Fehlstellungen die Wiederherstellung des 1. Strahles gewährleistet ohne die Wachstumsfugen zu verletzen. Der distale Gelenkflächenwinkel (DGW) muss bei der Korrektur des Hallux valgus unbedingt beachtet werden um ein frühzeitiges Rezidiv zu vermeiden
