Évaluation des pratiques de prescription de chimiothérapies anticancéreuses à des stades métastatiques avancés : résultats d’une étude pilote

Abstract

The objective of this pilot study conducted on 40 patients treated for advanced cancer was to describe drug prescribing, and to analyze the basis of decisions of chemotherapy and the benefit for the patients. The results of the interviews with prescribers have shown that when the treatment decision was taken, 74% of patients were in good general condition (0 < PS < 1) and 87% of them had already responded to the chemotherapy in previous treatments. Patients were informed and participated in the decision. The choice of treatments prescribed was based on an extrapolation of results obtained about the same tumor location at an earlier stage. The results obtained during the second phase of the study (follow-up to 3 years) confirmed the relevance of this decision, since 53% of patients responded to the nth-line chemotherapy. The median survival calculated on 26 patients was 15 months.

L’objectif de cette étude menée en deux étapes sur 40 patients traités pour cancer à des stades avancés était de décrire les pratiques de prescription, de rechercher les arguments intervenants dans les décisions de traitement et de déterminer le service rendu aux patients. Vingt-sept protocoles différents ont été prescrits, et dans 60 % des cas, les prescriptions étaient hors AMM. Les résultats des entretiens effectués avec les prescripteurs ont montré qu’au moment de la décision de traitement 74 % des patients étaient en bon état général (0 < PS < 1) et que 87 % d’entre eux avaient déjà répondu à la chimiothérapie lors de traitements antérieurs. D’une manière générale, les patients ont été informés et ont participé à la décision. Le choix des traitements prescrits était fondé sur une extrapolation des résultats obtenus dans lamême localisation tumorale à un stade plus précoce. Les résultats obtenus lors de la seconde étape de l’étude (suivi à trois ans) ont confirmé la pertinence de cette décision puisque 53 % des patients ont répondu à cette nième ligne de chimiothérapie. La médiane de survie calculée sur 26 patients était de 15mois.
