Management of oesophageal interventional endoscopic complications

Résumé

Les progrès de l’endoscopie interventionnelle oesophagienne permettent de proposer de plus en plus fréquemment des techniques de traitement endoscopique pour des pathologies oesophagiennes, aussi bien dans le domaine des lésions superficielles néoplasiques ou prénéoplasiques que des troubles moteurs ou des sténoses malignes ou bénignes. L’hémorragie immédiate ou retardée et la perforation restent les complications les plus fréquemment observées au cours de ces procédures. Certaines techniques ont des complications plus spécifiques qui doivent être parfaitement connues avant la procédure. Les patients doivent être informés de façon claire de ces risques. Ces risques doivent être anticipés, et surtout l’endoscopiste doit connaître leurs moyens de prise en charge. Plusieurs de ces complications peuvent être actuellement maîtrisées par des moyens médicaux et endoscopiques. Il faut néanmoins aussi connaître les situations nécessitant un recours à la chirurgie.

Abstract

Advances in interventional endoscopy involve more and more frequently the treatment of oesophageal diseases, in early oesophageal cancer, achalasia, or malignant or benign strictures. Bleeding and perforation are the most common complications observed during these proceprocedures. Some specific endoscopic techniques dedicated for complications management should be mastered before the procedure. Patients should be informed about potential complications. These risks must be anticipated. Many of these complications can be currently controlled by medical and endoscopic means. The use of surgical treatment should be recognized when necessary.
