The risk of colorectal neoplasia in cases of a family history of colorectal carcinoma or adenoma. Should the screening guidelines be revised in 2014?

Résumé

Depuis 2007, les données concernant l’augmentation du risque de cancer colorectal (CCR) et d’adénome « avancé » chez les parents au premier degré de personnes atteintes de CCR ou d’adénome ont été confirmées et enrichies, sans devoir déboucher sur des modifications majeures du dépistage actuellement proposé. Les indications qui datent de 2004 en France doivent rester suffisamment simples pour être appliquées chaque jour. En pratique quotidienne, chaque gastroentérologue doit faire l’effort de définir mieux, pour chaque patient, les données concernant le cas index pour individualiser le risque de CCR ou d’adénome : un sujet d’amélioration des pratiques professionnelles et révision de nos recommandations en 2014.

Abstract

Since 2007, data concerning an increase in the risk of colorectal cancer (CRC) and “advanced” adenoma in parents with a first degree relationship with patients with CRC or adenoma has been confirmed and developed, without impacting on the major changes in screening that is currently proposed. Indications that date back to 2004 in France must remain sufficiently simple so they can be applied each day. In daily practice, for every patient each gastroenterologist should make the time to better define the data concerning the index case to personalise the risk of CRC or adenoma: a matter of improving professional practices and revising our guidelines in 2014.
