Résumé
Ce court article pose la question de savoir si le pathologiste va continuer de jouer un rôle central dans le diagnostic des lésions mucosales du tractus gastrointestinal ou bien si son rôle sera repris par ses collègues cliniques équipés d’endomicroscopes de haute résolution. Pour supporter la raison d’être du pathologiste — au moins pour le présent et le futur proche —, l’auteur avance trois arguments: (i) les différences d’orientation du plan de vue (histologie: une vue perpendiculaire à la surface de la muqueuse vs. endoscopie: parallèle à la surface de la muqueuse); (ii) les avantages de pouvoir effectuer des colorations spéciales/des immunomarquages; (iii) la possibilité d’effectuer des coupes en profondeur et de consulter un collègue en cas de diagnostic difficile.
Abstract
This short review addresses the question whether pathologists will continue to play a central role in the diagnosis of mucosal lesions of the gastrointestinal tract or whether their role will soon be assumed by clinical colleagues equipped with modern high-resolution endoscopes. In order to support the raison d’être of the pathologist — at least for the time being and the near future — the author lists three arguments, related to (i) the differences in the orientation of the plane of view (histology: perpendicular to the mucosal surface vs. endoscopy: parallel to the mucosal surface), (ii) the advantages of staining and immunostaining tissue sections, and (iii) the possibility to perform deeper sections and to consul with colleagues in the case of difficult diagnoses.
