Zusammenfassung
Ein 55-jähriger Patient wird bewusstlos auf dem Bett liegend vorgefunden. Auffallend sind eine einseitige Mydriasis sowie Kreislaufinstabilität mit marmorierter Haut und Pulsen, die allseits nicht sicher palpabel sind. Im weiteren Verlauf wird fremd-anamnestisch herausgefunden, dass es im Rahmen eines Brustschmerzereignisses zu einer Synkope kam. In der Klinik werden mittels transthorakaler Echokardiographie ein hämodynamisch relevanter Perikarderguss sowie Hinweise auf eine akute Aortendissektion festgestellt. Der Fall zeigt die Notwendigkeit, bei einem neu aufgetretenen neurologischen Defizit, an eine Aortendissektion zu denken.
Abstract
A 55-year-old man was found unconscious on his bed. Irregular was unilateral mydriasis, hemodynamical instability causing marbled skin and unpalpable pulses. Anamnesis showed that there was syncope during a previous event of chest pain. In the hospital, a transthoracic echocardiogram revealed hemodynamically relevant pericardial effusion and high-grade suspicion of acute aortic dissection. It is evident in this case that it is important to consider aortic dissection for a new-onset neurological deficit.
