Zusammenfassung
Eine 19‑jährige Patientin wurde mit Vigilanzminderung (GCS 5) nach einem Diskobesuch aufgefunden und kreislaufstabil in die Notaufnahme gebracht. Das Urin-Drogenscreening war positiv auf Amphetamine. In der CT-Untersuchung des Schädels konnte kein Hinweis auf eine Blutung oder Ischämie als Ursache der Vigilanzminderung gefunden werden. Laborchemisch zeigte sich eine hypotone Hyponatriämie (118 mmol/l), welche mit 3 %iger Kochsalzlösung therapiert wurde. Nach Extubation berichtete die Patientin, dass sie Ecstasy (MDMA, 3,4-Methylendioxy-N-methylamphetamin) eingenommen hatte. In der Literatur finden sich mehrere Fallberichte über schwere Hyponatriämien nach MDMA-Einnahme, teilweise mit Hirnödem und tödlichem Ausgang.
Abstract
A 19‑year-old comatose patient (GCS 5) was brought to the emergency room after visiting a club the night before. Urine toxicology screen was positive for amphetamine. The cranial CT scan showed no signs for bleeding or ischemia. Lab tests documented hypotonic hyponatremia (118 mmol/l). The patient was admitted to the intensive care unit and treated with 3% sodium chloride infusion. After extubation, the patient reported consumption of ecstasy (MDMA, 3,4-methylenedioxymethamphetamine). There are several case reports documenting severe hyponatremia after MDMA consumption, some with cerebral edema and fatal outcome.
