Zusammenfassung
Hintergrund
Spätestens seit der Publikation der „Handlungsempfehlungen für das präklinische Atemwegsmanagement“ der wissenschaftlichen Arbeitskreise Notfallmedizin und Atemwegsmanagement der Deutschen Gesellschaft für Anästhesiologie und Intensivmedizin stellt die nichtinvasive Ventilation (NIV) in der Präklinik eine praktikable Option im Therapiealgorithmus „respiratorische Insuffizienz“ dar. Gleichwohl fehlt es noch immer an umfassender Evidenz, die den präklinischen Einsatz einer NIV absichert, sodass bislang häufig die innerklinische S3-Leitlinie „Nichtinvasive Beatmung als Therapie der akuten respiratorischen Insuffizienz“, zuletzt 2015 aktualisiert, herangezogen wird.
Ziel der Arbeit
Ziel der hier vorliegenden Übersichtsarbeit ist es, mithilfe einer aktuellen medizinischen Datenbankanalyse, Möglichkeiten und Grenzen einer präklinischen NIV zu beschreiben und den Lesern einen Überblick über die aktuelle Daten- und Evidenzlage zu bieten.
Material und Methoden
Systematische Auswertung elektronischer medizinischer Datenbanken (Pubmed, Cochrane Library) unter Verwendung der Termini „NIV“, „ventilation“, „pneumonia“, „cardiogenic (o)edema“, „COPD“, „Asthma“, „heart failure“, „trauma“, „palliative care“, „immunosuppression“, „pre(-)hospital“, „pre(-)clinical“ sowie „out(-)of(-)hospital“ sowie Referenzlisten im Zeitraum 2006 bis 2016.
Ergebnisse
Im Untersuchungszeitraum wurden insgesamt 81 Arbeiten identifiziert, die sich mit der Anwendung präklinischer NIV bei unterschiedlichen Krankheitsbildern beschäftigte. Die weitaus umfangreichsten Daten existieren zur Anwendung bei kardiogenem Lungenödem sowie bei akut exazerbierter „chronic obstructive pulmonary disease“ (COPD). Daten aus großen randomisierten Arbeiten fehlen weiterhin.
Diskussion
Auch rund 15 Jahre nach den ersten Beschreibungen fehlt es an umfassender Evidenz, die den Einsatz präklinischer nichtinvasiver Ventilation auch wissenschaftlich absichert.
Abstract
Background
Following publication of the “Recommendations for Prehospital Airway Management” by the Task Force on Emergency Medicine and Airway Management of the German Association for Anesthesiology and Intensive Care, non-invasive ventilation (NIV) represents a viable treatment option in the prehospital algorithm for respiratory failure; however, there is no evidence to support the routine use of prehospital NIV. Currently, the most relevant available guidance is given in the S3 guidelines on NIV as treatment for acute respiratory failure, last updated in 2015.
Objective
The purpose of this review article is to describe the feasibility of NIV prior to admission to hospital and any potential limitations. This was achieved through a review of current medical databases.
Material and methods
Systematic evaluation of electronic medical databases (PubMed, Cochrane library) using the search terms “NIV”, “ventilation”, “pneumonia”, “cardiogenic (o)edema”, “COPD”, “asthma”, “heart failure”, “trauma”, “palliative care”, “immunosuppression”, “pre(-)hospital”, “pre(-)clinical” and “out(-)of(-)hospital” as well as reference lists in the period 2006–2016.
Results
The review identified a total of 81 studies in the study period, which were concerned with the utilization of prehospital NIV for a variety of symptomatic diseases. The most extensive data for prehospital NIV exist for its use in cardiogenic pulmonary edema and acute exacerbated chronic obstructive pulmonary disease (COPD); however, there were no data from large randomized controlled studies.
Conclusion
Approximately 15 years after the first descriptions there is still a lack of comprehensive evidence that would scientifically secure the use of prehospital NIV, despite its wide utilization.
