Zusammenfassung

Es wird über das Beinahe-Ertrinken eines einjährigen Jungen berichtet, der mit einer schweren Hypothermie aufgefunden wurde. Durch eine kontinuierliche Rettungskette vom Ersthelfer bis in den Schockraum des örtlichen Krankenhauses konnte nach 3 h ein Minimalkreislauf wieder hergestellt werden. Bei anhaltender Hypothermie, Verschlechterung der Oxygenierung und Schock wird eine ECLS (Extracorporal Life Support) zur Wiedererwärmung, effektiven Sauerstoffversorgung und Organperfusion durch ein Team des nächstgelegenen Herzzentrums initiiert. Mit Hilfe eines portablen ECLS-Systems wird der Junge in das nächstgelegene Herzzentrum verlegt. Nach 27 Tagen konnte das Kind in eine Klinik für Kinderrehabilitation entlassen werden.

Abstract

The article presents the case of a 1-year-old boy with near-drowning and severe hypothermia. Minimal circulation could be restored after 3 h by a continuous rescue chain. The resuscitation was started by a first responder, followed up by paramedics and an emergency physician, and the emergency room team in the local hospital. Due to a prolonged hypothermia, critical oxygenation, and shock, extracorporal life support (ECLS) for rewarming, effective supply of oxygen, and organ perfusion was initiated. With the support of the mobile ECLS, the boy was transferred to the university heart center. After 27 days the boy could be released to a center of rehabilitation for children.
