Zusammenfassung
Patientenrettungen aus exponierter Lage stellen eine medizinisch wie technische Herausforderung dar. Die Arbeitssicherheit der beteiligten Einsatzkräfte steht hierbei im Fokus. Wir berichten von einem 52-jährigen Dachdecker mit Herzkreislaufstillstand in 14 m Höhe auf einer im Bau befindlichen Dachkonstruktion in einem Hinterhof. Trotz initialer ROSC („return of spontaneous circulation“) kommt es bei protrahierter hämodynamischer Instabilität zu einem erneuten Herzkreislaufstillstand. Nach Ausschöpfen aller prähospitalen Behandlungsoptionen wird die Entscheidung zum Transport unter automatisierter kardiopulmonaler Reanimation getroffen. Die folgende Kasuistik schildert die komplexe technische Rettung mittels LUCAS™ 2. Nachfolgend wird der Einsatz von mechanischen Reanimationshilfen sowie die Indikation für den Transport unter laufender Reanimation kontrovers diskutiert.
Abstract
Rescue of patients from difficult locations is technically complex. Safety of work for the recue team is of utmost importance. This case report presents a 52-year-old construction worker with cardiac arrest at a height of 14 m on a roof. Despite initial return of spontaneous circulation, the patient presented with hemodynamic instability and re-arrest. After extensive prehospital therapy, the decision was made to transport the patient using a mechanical cardiopulmonary resuscitation (CPR) device (LUCAS™ 2). In the case report, using mechanical devices for prolonged CPR is also critically discussed.
