Zusammenfassung
Hintergrund
Bei der Versorgung eines stark blutenden Patienten im Schockraum besitzen neben der kausalen Behandlung hämotherapeutische Maßnahmen einen hohen Stellenwert. Um auf Massivtransfusionen vorbereitet zu sein, soll für den Schockraum und das Blutdepot ein gemeinsames Konzept erstellt werden.
Methoden
Grundlage dafür sind die in der Literatur beschriebenen Besonderheiten der Notfall- und Massivtransfusion.
Ergebnisse
Für die notfallmäßige Bereitstellung der Blutkomponenten bedarf es eines nach Dringlichkeit abgestuften Vorgehens zur Blutgruppenbestimmung und Konservenauswahl. Bei Durchführung einer Massivtransfusion werden im Schockraum von Anfang an Erythrozytenkonzentrate und Frischplasmen blockweise in 4 + 4er-Paketen verabreicht. Gerinnungsfaktorenkonzentrate werden, insbesondere zur Behandlung einer Verdünnungskoagulopathie, initial dazugegeben.
Schlussfolgerung
Die Bereitstellung der bei Raumtemperatur lagerbaren Blutprodukte in einer Notfallkiste im Schockraum kann den Beginn einer Massivtransfusion vereinfachen.
Abstract
Background
For the treatment of massively bleeding patients in the emergency room hemotherapy is of great importance in addition to causal therapy. To be prepared for a massive transfusion a common concept has to be made for the emergency room and for the laboratory.
Methods
The basic principles for forming this concept are the special features of urgent transfusions and massive transfusions reported in the literature.
Results
For issuing of blood components in emergency situations a plan for blood grouping and selecting the blood products considering the urgency is needed. In the emergency room red blood cell concentrates and fresh plasma are given in 4 + 4 packages from the beginning. Initially coagulation factor concentrates are added especially for treatment of dilution coagulopathy.
Conclusions
Providing blood products that can be stored at room temperature in a box in the emergency room can simplify the start of a massive transfusion.
