Zusammenfassung
Hintergrund
Die Ursachen eines Lungenödems können unterschiedlichster Genese sein. Im dargestellten Fall kam es bei einer 39-jährigen Patientin zu einer hypertensiv bedingten kardialen Dekompensation mit konsekutivem Lungenödem aufgrund eines bis dahin nicht diagnostizierten Conn-Syndroms.
Therapie und Verlauf
Die notärztlich eingeleitete diuretische und antihypertensive Therapie konnte die klinische Situation der Patientin nicht verbessern, sodass nach weiterer Dekompensation die endotracheale Intubation mit kontrollierter Beatmung unumgänglich wurde. In der Notaufnahme zeigten sich in der initialen Blutgasanalyse eine metabolische Alkalose und ein Kaliumwert von 1,7 mmol/l, die pathognomisch für ein Conn-Syndrom sind. Die Magnetresonanztomographie zeigte eine Raumforderung im Bereich der rechten Nebennierenrinde. Durch die intensivmedizinische Therapie konnte die klinische Situation verbessert werden, sodass die Patientin nach 24 h extubiert wurde.

Pulmonary edema can have various origins. In the illustrated case, a 39-year-old woman suffered from hypertension-induced cardiac decompensation with pulmonary edema due to previously undiagnosed Conn syndrome.
Therapie and clinical course
Pulmonary edema can have various origins. In the illustrated case, a 39-year-old woman suffered from hypertension-induced cardiac decompensation with pulmonary edema due to previously undiagnosed Conn syndrome. The initiated diuretic and antihypertensive therapy did not improve the clinical situation. After further decompensation, endotracheal intubation with controlled ventilation was necessary. The initial blood gas analysis in the emergency room showed metabolic alkalosis and severe hypokalemia (1.7 mmol/l), pathognomic for Conn syndrome. Magnet resonance imaging showed a tumor in the right adrenal cortex. The clinical situation improved due to the pharmacological intensive care therapy. The patient could be extubated after 24 h.
