Anamnese und Diagnose
Ein 4-jähriges Kind und dessen Mutter erlitten eine schwere Kohlenmonoxid- (CO-)Vergiftung durch eine defekte Gastherme im Badezimmer. Wir berichten, wie das initial bewusstlose und krampfende Kind erfolgreich mit einer hyperbaren Sauerstofftherapie behandelt wurde. Nach erschwerter Anamnese konnte die Diagnose „CO-Intoxikation“ schließlich präklinisch durch ein CO-Oxymeter gestellt werden.
Therapie
Nach Narkoseeinleitung und Intubation wurde das Kind auf eine pädiatrische Intensivstation gebracht. Von dort wurde es in ein Druckkammerzentrum verlegt, wo die HBO-Therapie nach 3,75 h begonnen wurde. Am nächsten Morgen war das Kind neurologisch unauffällig. Es konnte nach 4 Tagen nach Hause entlassen werden.

Anamnesis and diagnosis
A 4-year-old child and the mother suffered severe carbon monoxide (CO) poisoning due to a defective gas water heater in the bathroom. We describe how the child who was unconscious and suffering from seizures was successfully treated with hyperbaric oxygenation (HBO). The reason for unconsciousness remained initially unresolved due to a difficult anamnesis but CO poisoning was finally diagnosed by the preclinical use of CO oximetry.
Therapy
After induction of narcosis and intubation the child was transported to the intensive care unit (ICU) of a children’s hospital. From there, the child was transferred to a hyperbaric chamber, where hyperbaric oxygenation started after 3.75 h. The following morning the child showed no neurological symptoms and could be safely discharged after 4 days.
