Zusammenfassung

Anamnese

Ein 53-jähriger Patient mit einer langjährig bekannten schizophrenen Psychose scheint erneut einen akuten psychotischen Schub zu erleiden. Der Notarzt findet den Mann in dessen Wohnung nackt auf einem Stuhl sitzend vor und weist ihn in die betreuende Psychiatrie ein. Hier kommt es rasch zur kardiopulmonalen Dekompensation. Während der anschließenden Notfallverlegung manifestiert sich zusätzlich ein akutes Abdomen. Beim Eintreffen in der Zielklinik hatte der Mann außerdem eine deutliche Pupillendifferenz und ein Troponin-positives akutes Koronarsyndrom entwickelt.

Diagnose und Therapie

Die notfallmäßig durchgeführte Laparatomie zeigt eine Peritonitis nach Magenperforation und macht die Sepsis als Hauptursache für die beobachteten klinischen Symptome wahrscheinlich. Nach Laparatomie, Koronarintervention und Therapie der Sepsis mit Multiorgandysfunktion konnte der Patient nach 20 Tagen Krankenhausaufenthalt entlassen werden.

Abstract

Anamnesis

A 53-year-old patient with known schizophrenia apparently suffered an acute aggravation of the underlying disease. The emergency physician found the patient in his apartment sitting naked on a chair and subsequently committed him to supervision psychiatry where he rapidly developed cardiopulmonary insufficiency. During the subsequent emergency transfer to the university clinic the patient started to show signs of an acute abdomen. On arrival at hospital the patient additionally developed anisocoria and troponin positive acute coronary syndrome (ACS).

Diagnosis and therapy

Emergency laparotomy revealed peritonitis due to gastric perforation making sepsis the probable main cause of the observed clinical symptoms. After abdominal surgery, cardiac catheterization and treatment of severe sepsis with multiorgan dysfunction syndrome, the man was discharged after 20 days in hospital.
