Zusammenfassung

Die intraaortale Ballonpumpe (IABP) führt zu einer Verbesserung der Hämodynamik bei Patienten mit kardiogenem Schock. Nach der derzeitigen Datenlage ist eine Anwendung bei Patienten ohne frühzeitige perkutane Koronarintervention (PCI) oder geplante operative Revaskularization sowie bei Patienten mit mechanischen Komplikationen des akuten Herzinfarkts zu empfehlen. Sie sollte möglichst frühzeitig erfolgen. Inwieweit eine generelle Anwendung der IABP bei frühzeitiger primärer PCI sinnvoll ist, wird derzeit in der IABP-Shock-II-Studie geprüft.

Abstract

The use of intra-aortic balloon pumps (IABP) leads to an improvement in the hemodynamics in patients with cardiogenic shock. According to the current state of knowledge the use is recommended for patients without timely percutaneous coronary intervention (PCI) or planned operative revascularization and patients with mechanical complications of acute heart attack. The IABP should be implanted as early as possible after onset of shock. Wether a general use in all patients with shock treated with primary PCI is currently under investigation in the IABP Shock II trial.
