Zusammenfassung
Supraglottische Alternativen zu Maskenbeatmung und endotrachealer Intubation stellen einen wichtigen Bestandteil moderner Konzepte zur Atemwegssicherung im Notfall dar. Während die Funktion entsprechender Hilfsmittel als Rückfallebene bei Intubationsschwierigkeiten breit akzeptiert ist, müssen die Vorteile supraglottischer Atemwege im Vergleich zur Beatmung mit der Gesichtsmaske unter Notfallbedingungen vermehrte Beachtung finden. Sinnvoll ist die Festlegung regionaler Konzepte, um die Sicherheit der Anwendung zu steigern und die Schulungsmaßnahmen zu minimieren. Aufgrund der zunehmenden Verbreitung und Anwendung geeigneter supraglottischer Alternativen in der Notfallmedizin sollten Konzepte für das sichere Überführen in einen Endotrachealtubus etabliert sein.
Abstract
Supraglottic alternatives to mask ventilation and tracheal intubation are part of modern concepts for emergency airway management. While there is general acceptance of their role as a backup in case of intubation difficulties, more attention should be paid to the advantages of supraglottic devices over ventilation with a facemask in an emergency setting. Establishing regional concepts allows safe use and reduces training requirements. Due to the growing availability and use of suitable supraglottic alternatives in emergency medicine, concepts for the conversion to a tracheal tube should be established.
