Zusammenfassung
Akute Dyspnoe im Rahmen obstruktiver Atemwegserkrankungen, wie der akute Asthmaanfall und die Exazerbation der chronisch obstruktiven Lungenerkrankung, stellt eine der häufigsten Notfälle in der Akutmedizin dar. Nach differenzialdiagnostischer Abgrenzung gegenüber anderen Ursachen (kardiale Dekompensation, Pneumonie, Lungenembolie, Pneumothorax, Fremdkörperaspiration etc.) besteht das Hauptziel der initialen Therapie in der Aufrechterhaltung der Oxygenierung. Therapeutika der ersten Wahl sind β2-Sympathomimetika, Kortikosteroide, Parasympatholytika und Sauerstoff. Wenn durch diese Maßnahmen keine Stabilisierung sichergestellt werden kann, stehen weitere Therapeutika (Magnesium, Lidocain, Theophyllin, volatile Anästhetika etc.) zur Verfügung, bis schließlich auf eine Form der mechanischen Unterstützung der Ventilation (invasiv oder nicht-invasiv) zurückgegriffen werden muss.
Abstract
Acute dypnoea due to an acute asthma attack or exacerbation of chronic obstructive pulmonary disease is a frequent cause of emergency treatment. Alternative diagnosis can be acute left heart failure with pulmonary edema, pneumonia, lung emboli, pneumothorax, and foreign body aspiration. Cornerstones of acute treatment are β2 adrenergic agonists, systemic corticosteroids and parasymaptholytics to maintain oxygenation. Additional pharmacological options are magnesium, volatile anesthetics, lidocaine, ketamine, and propofol. Finally, depending on the clinical development a mechanical assist of ventilation might be necessary in term of invasive or non-invasive ventilation.
