Zusammenfassung
Penetrierende Thoraxverletzungen sind bei Kindern eine Rarität und meist Folge von Unfällen. Wir berichten den Fall einer kindlichen, thorakalen Stichverletzung mit Verletzung der Aorta und Herzbeuteltamponade. Das präklinische Management und unser Therapieregime bei penetrierenden Brustkorbverletzungen in einem Haus der Schwerpunktversorgung (Traumazentrum ohne Thoraxchirurgie) werden dargestellt. Unter Reanimationsmaßnahmen erfolgte eine „emergency department thoracotomy“ (EDT). Im Rahmen engmaschiger Verlaufskontrollen konnte eine restitutio ad integrum dokumentiert werden.
Abstract
Penetrating chest injuries in childhood are extremely rare and in most cases due to an accident. We report the case of a 14-year-old boy with a thoracic stab wound leading to aortic damage and pericardial tamponade. On admission he was in shock and an immediate emergency department thoracotomy (EDT) was performed under cardiopulmonary resuscitation. We report our treatment regime for thoracic injuries and the specific management of this patient. Subsequent close control and surveillance resulted in full recovery of the patient.
