Zusammenfassung

Das Thoraxtrauma ist mit einer Häufigkeit von 45% eine der häufigsten Verletzungen bei polytraumatisierten Personen. Eine kausale Behandlung zur Vermeidung und Prävention von Folgeschäden ist oft möglich und bei korrekter Diagnosestellung durchführbar. Dem Notarzt stehen während der präklinischen Phase der Behandlung allerdings wenig diagnostische Möglichkeiten zur Verfügung. Ziel der vorliegenden Arbeit ist es, einen Überblick über den aktuellen Wissensstand zur Vorgehensweise bei der Notfallversorgung von thorakal verletzten Patienten zu verschaffen. Dazu erfolgte eine umfangreiche systematische Literatursuche zur detaillierten Betrachtung einzelner pathophysiologischer Probleme und zur Erlangung neuer Erkenntnisse durch klinische und experimentelle Studien. In diesem Artikel werden die lebensbedrohlichen und primär behandlungspflichtigen Verletzungen nach neusten Erkenntnissen beschrieben. Der Beitrag soll dem jungen und dem erfahrenen Notarzt als Hilfestellung und Entscheidungshilfe im Umgang mit derartigen Verletzungen dienen.

Abstract

Chest trauma is a frequent injury (45%) in polytraumatized patients. A causal and appropriate treatment to avoid long-term-consequence is necessary and possible. Due to the reduced diagnostic options the preclinical treatment seems to be a difficult assignment. The aim of this article is to give an overview over the present knowledge about the therapy of chest injured patients. We did a bulked systemic literature research with detailed regard on pathophysiologic problems and new cognitions from clinical and experimental studies. The article describes life threatening traumata and their treatment considering the newest knowledge, it is created for young emergency medicines and those who already have experience in the field of polytrauma treatment to provide a guideline and decision development.
