Zusammenfassung
Endokrinologische Notfälle sind polysymptomatische Krankheitsbilder mit Beeinträchtigung multipler Organsysteme. Meist entstehen sie auf dem Boden chronischer Erkrankungen unter dem Leitsymptom neurologischer Funktionsstörungen bis hin zum Koma in Kombination mit vielfältigen Organfunktionsstörungen. Sie können somit als Chamäleon der Inneren Medizin bezeichnet werden. Zeigt ein Notfallpatient neben neurologischen oder psychiatrischen Funktionsstörungen gleichzeitig multiple Organsymptome, so besteht differenzialdiagnostisch der begründete Verdacht auf eine akute Endokrinopathie. Endokrinologische Notfälle sind meist protrahiert im Verlauf, sie machen ca. 3% der Notarzteinsätze mit Klinikeinweisung aus. Unter den Endokrinopathien stellen die Hypo- bzw. Hyperglykämie das mit Abstand höchste Einsatzaufkommen im Notarztdienst dar, sie sind deshalb im nachfolgenden Artikel entsprechend gewichtet. Akute Endokrinopathien der Hypophyse, Schilddrüse, Nebennieren und des Kalziumstoffwechsels spielen insbesondere im Notarztdienst zahlenmäßig eine untergeordnete Rolle, sollten aber bei unklarer Situation immer differenzialdiagnostisch in Betracht gezogen werden. Aufgrund des bunten Bildes der Symptome ohne eindeutige Leitsymptome besteht ein hohes Fehleinschätzungspotenzial.
Abstract
Metabolic emergencies are polysymptomatic disease patterns accompanied by the impairment of multiple organs. They mostly occur on the basis of chronic evil under the guiding symptom of brain dysfunction as far as coma in combination with multiple organ dysfunctions. They can therefore be seen as the chameleons of internal medicine. In case of neurological or psychiatric symptoms with simultaneous occurring of multiple signs of organ malfunction, the combination strongly indicates an acute metabolic disorder. A metabolic emergency does not occur all on a sudden but develops over time and is often difficult to recognize. 3% of all emergencies that require hospitalization are metabolic disorders. Hypo-and hyperglycemias are by far the most common cause of emergencies among metabolic disorders, and are the reason why this article focuses so much on this subject. Acute metabolic emergencies involving hypopyhsis, thyreoid gland, suprarenal gland and calcium metabolism are relatively rare, but should nevertheless be considered, if the situation is unclear. There is a high danger of symptoms being wrongly interpreted, due to the fact that most of these emergencies lack typical principal signs. Nevertheless one can avoid sending people to the psychiatric hospital because of “hypo”, recognizing the major symptoms of metabolic disorders.
