Zusammenfassung
Die Behandlung maligner Arrhythmien mit implantierbaren Defibrillatoren (ICD) gehört mittlerweile zu einer etablierten Therapie, die sowohl primär als auch sekundär präventiv eingesetzt wird. Durch technische Weiterentwicklung und nicht zuletzt durch leistungsstärkere Mikroprozessoren wurden die verfügbaren Therapieoptionen der ICD immer differenzierter. So sind aktuell Ein-, Zwei- und auch Dreikammersysteme verfügbar. Auch die Therapieoptionen sind kontinuierlich weiter verfeinert worden. So können sich aus Interaktion der verschiedenen Elektroden untereinander nicht immer leicht zu durchschauende Fehlfunktionen ergeben. Sämtliche aktuell verfügbare ICDs sind zusätzlich mit einem Schrittmachersystem versehen, das in Abhängigkeit von seiner Stimulationshäufigkeit ebenfalls zu Problemen führen kann, die im Rettungsdienst korrekt erkannt und behandelt werden müssen. Anhand konkreter Beispiele werden häufige Fehlfunktionen von ICDs vorgestellt. Das Verhalten in typischen Situationen wird mit den entsprechenden Konsequenzen besprochen.
Abstract
The implantable defibrillator (ICD) is a well established therapy for the primary as well as the secondary prophylaxis of malignant ventricular arrhythmia. Technical advances, in particular the use of powerful microprocessors, have allowed for more differentiated therapy options. There are currently one, two and also three chamber systems available. These therapy options have also been continuously developed. Thus, interactions between the various electrodes can lead to difficult to find malfunctions. All currently available ICDs have an additional pacemaker system which can also lead to problems dependent on its stimulation frequency. These must be recognized in emergency situations. Frequent ICD malfunctions are discussed using concrete examples. The proper reactions in typical situations with corresponding consequences are discussed.
