Zusammenfassung
Chronisch ventilatorische Insuffizienz (CVI) bzw. Hypoventilation wird verursacht durch die Insuffizienz der Atemmuskulatur, d. h. der Atempumpe. Die CVI geht mit einer Erhöhung des PaCO2 einher. Die Hyperkapnie im Schlaf und unter Belastung ist ein Frühsymptom. Das Ausmaß der Hyperkapnie ist ein Index für den Schweregrad der CVI. Wesentliche Krankheitsgruppen, die häufig zur CVI führen, sind die chronisch obstruktive Lungenerkrankung (COPD), Thorakorestriktion, Obesitas-Hypoventilation und neuromuskuläre Erkrankungen. Steht als wesentliche Ursache der CVI die erschöpfte Atmungsmuskulatur im Vordergrund, ist die Heimbeatmung in Form der nichtinvasiven Maskenbeatmung die Therapieform der Wahl.
Mit zunehmender Verbreitung der Heimbeatmung muss sich in Zukunft auch die Notfallmedizin mit diesem Thema beschäftigen. Bei Patienten mit Heimbeatmung entweder über Maske oder über ein Tracheostoma, die komplett oder weitgehend ventilatorabhängig sind, kann es infolge technischer Defekte des Beatmungsgeräts, akuter Exazerbationen oder Progredienz der Grunderkrankung unter häuslichen Bedingungen zu respiratorischen Notfallsituationen kommen. Eine Reihe von Sofortmaßnahmen werden im Artikel beschrieben. Nach erfolgter Notfallversorgung sollte die zügige Kontaktaufnahme mit dem zuständigen Beatmungszentrum erfolgen. Besonders in der Endphase der Erkrankung bei Patienten mit Heimbeatmung stellen sich auch für die Notfallmedizin ethische Fragen.
Abstract
Chronic ventilatory insufficiency (CV) or hypoventilation is caused by insufficiency of the respiratory muscles, i. e. the respiratory pump. CVI is associated with an increase in PaCO2. Hypercapnia during sleep and exercise is an early symptom. The extent of hypercapnia is an index of the severity of the CVI. The following diagnosis frequently lead to CVI: chronic obstructive pulmonary disease, thoracic restriction, obesity hypoventilation and neuromuscular diseases. In the case of respiratory muscle fatigue, home mechanical ventilation in terms of both invasive and non-invasive mechanical ventilation is a first choice therapy.
With increasing prevalence of home mechanical ventilation, it becomes necessary for emergency medicine to focus on this topic. For patients utilizing home mechanical ventilation, either with a mask or via tracheostoma, who are largely or fully ventilator dependent, technical defects of the ventilator can lead to a respiratory emergency. Furthermore, acute exacerbation or progression of the underlying disease may also cause an emergency at home. Procedures of troubleshooting in various situations are discussed in this article. Aiming at optimal care of the patient the responsible center should be contacted immediately after the intervention. Ethical questions arise for emergency treatment of patients with home mechanical ventilation in the end stage of illness.
