Zusammenfassung
Fragestellung: Häufigkeit und Verteilung chirurgischer Krankheitsbilder bei Kindern, die zu einem akuten Abdomen führen und operiert werden müssen. Methode: Retrospektive Analyse der Operationsberichte im Dr. von Haunerschen Kinderspital im Jahr 2003 mit Erfassung der intraoperativen Diagnose und des Alters der Patienten. Erfassen der interdisziplinären Zusammenarbeit durch Analyse der Häufigkeit kinderchirurgischer Konsile in der allgemeinpädiatrischen Notfallambulanz. Ergebnisse: Von 5203 Operationen, die im Jahr 2003 durchgeführt wurden, entfielen 132 (2,5%) auf akute abdominelle Probleme. Die häufigste Ursache eines akuten Abdomens war die Appendizitis mit Häufigkeitsgipfel im Schulalter. Für die anderen Erkrankungen finden sich ebenfalls typische Altersverteilungen. Malignome als Ursache eines akuten Abdomens sind im Kindesalter selten. Bei 9822 ambulanten pädiatrischen Behandlungen in der Notfallambulanz im Jahr 2003 war bei 125 Patienten (1,3%) ein kinderchirurgisches Konsil erforderlich, davon wurde bei 25 Patienten (20%) die Indikation zur Operation gestellt. Schlussfolgerungen: Das akute Abdomen beim Kind als nicht planbare Operationsindikation stellt einen relativ großen Anteil der gesamten durchgeführten Operationen. Die interdisziplinäre Zusammenarbeit zwischen Kinderärzten und Kinderchirurgen bei akuten abdominellen Beschwerden führt in etwa einem Fünftel der Fälle zur Operation.
Abstract
Objective: The aim of the study was to determine frequency and distribution of disease patterns in children that lead to acute abdomen requiring surgical intervention. Method: Surgical reports from the Dr. von Haunerschen Children’s Hospital for 2003 were retrospectively analyzed including intraoperative diagnosis and patient’s age. Analysis of the frequency of consultation with the Children’s Surgical Department in the General Pediatric Outpatient Emergency Ward yielded data on interdisciplinary cooperation. Results: Of 5203 operations performed in 2003, 132 (2.5%) pertained to acute abdominal problems. The most frequent cause of acute abdomen was appendicitis peaking in frequency during school age. The other diseases also exhibited typical age distribution patterns. Malignant tumors as the cause of acute abdomen are rare in childhood. Of 9822 pediatric outpatients treated in the emergency unit in 2003, consultation with a pediatric surgeon was necessary in 125 patients (1.3%); among these surgery was indicated in 25 patients (20%). Conclusions: Acute abdomen in children as a surgical indication that cannot be planned accounts for a relatively large proportion of all operations performed. Interdisciplinary cooperation between pediatricians and pediatric surgeons in patients with acute complaints of abdominal pain leads to surgery in approximately one-fifth of the cases.
