Zusammenfassung
Seit den 1970er Jahren werden Anstrengungen unternommen, auch außerklinisch die Defibrillation bei Herzstillstand mit Kammerflimmern zu einem möglichst frühen Zeitpunkt anzuwenden. Die Entwicklung von automatisierten externen Defibrillatoren (AED), die durch Analysealgorithmen eine Diagnosestellung durch den Anwender verzichtbar machen, erlaubte die weitere Verbreitung von Frühdefibrillationsprogrammen. So wurde 1989 in München als 2. deutscher Großstadt das erste bayerische Frühdefibrillationsprogramm unter Beteiligung aller Hilfsorganisationen etabliert. Aufgrund eines Gutachtens des Bayerischen Staatsministerium des Innern von 1995 wurde in Bayern die Zielsetzung definiert, die landesweite Frühdefibrillation durch Rettungsdienstpersonal als Standard in die rettungsdienstliche präklinische Routine einzuführen. Ein 2001 vorgelegtes Konzept des Ministeriums regelt Rahmenbedingungen und Qualitätssicherung der Frühdefibrillationsprogramme innerhalb des Rettungsdienstes. 2003 wurde ein adaptiertes Konzept für den Berg- und Wasserrettungsdienst in Bayern entwickelt. Auch nicht an dieses Konzept gebundene, wie beispielsweise innerbetriebliche Programme richten sich weit überwiegend eng an diesen Vorgaben aus. Eine Machbarkeitsstudie beschäftigt sich zurzeit mit der Umsetzung der sog. Public Access Defibrillation (PAD) in Bayern.
