Abstract
Successful use of automatic external defibrillators in the United States prompted a pilot study in Berlin published in 1989. This led to systematic introduction of early defibrillation in Berlin and comparable concepts in different German cities. Various successful studies and further technical development of lighter and less expensive equipment resulted in expansion of the concepts in the sense of first responder defibrillation or public access defibrillation. Nevertheless, several unsolved problems remain: the question of the most effective shock form and necessary amount of energy has not been resolved nor the question of how intensive training for early defibrillation should be beyond the scope of emergency medical services. These questions should be further analyzed in ongoing studies.
