Zusammenfassung
Die Sicherung der Atemwege durch endotracheale Intubation und Beatmung ist für den Notfallpatienten von vitaler Bedeutung. Die fehlerhafte Platzierung des Tubus, sei es durch einseitige Intubation eines Bronchus oder durch Tubusfehllage im Ösophagus, führt zur Hypoxie und schweren Komplikationen wie Aspiration, Magenruptur, Pneumonie oder Spannungspneumothorax. Daher muss nach einer Intubation die korrekte Lage des Tubus durch klinische Verfahren, insbesondere durch Auskultation überprüft werden. Als technische Hilfsmittel zur Überprüfung der endotrachealen Tubuslage können Esophageal Detector Device oder Kapnometrie bzw. -graphie die klinische Untersuchung bestätigen, sodass die korrekte Tubuslage stets durch zwei unabhängige Verfahren abgesichert werden sollte. Beide Methoden liefern auch im präklinischen Bereich eine ausreichende diagnostische Sicherheit, um die genannten Risiken für den Patienten abzuwenden.
Abstract
Maintaining patent airways with endotracheal intubation and ventilation is vitally important for emergency patients. Incorrect placement of the tube, be it unilateral intubation of one bronchus or incorrect positioning in the esophagus, leads to hypoxia and serious complications such as aspiration, stomach rupture, pneumonia, or tension pneumothorax. For checking the placement of the endotracheal tube, technical resources such as an esophageal detector device, capnometry, or capnography could provide confirmation of the clinical examination, ensuring that correct tube placement is always verified by two independent procedures. Both methods afford sufficient diagnostic reliability to avoid the above-mentioned risks to the patient.
