Zusammenfassung
Pro Jahr erleiden in Deutschland akut etwa 1200 Personen eine Querschnittlähmung. In 25% der Fälle ist ein Arbeitsunfall die Ursache, 25–30% werden durch andere Grunderkrankungen ausgelöst. Die moderne Wirbelsäulenchirurgie – unter Nutzung minimalinvasiver Techniken oder mit Risikominimierung der Pedikelinstrumentation in Grenzgebieten durch die spinale Neuronavigation – bildet einen Baustein in der umfassenden, spezialisierten Behandlung Querschnittgelähmter. Eine kausale Behandlung zerrissener Rückenmarkstrukturen ist derzeit nach wie vor unmöglich. Behandelt werden die durch die neuronale Läsion entstehenden, lähmungsbedingten Komplikationen. Hypersekretion, thermische, vegetative und kardiale Dysregulationsstörungen bis hin zum Herzstillstand im spinalen Schock sind genauso lebensgefährlich wie die bis zu 6 Monaten nach dem Unfall gehäuft auftretenden Thrombosen und fulminanten Lungenembolien. Ziel einer Behandlung bei akut erlittener Querschnittlähmung ist immer eine rasche Mobilisation. Beispielsweise ist der Sinn einer Wirbelsäulenstabilisierung hinfällig, wenn dadurch die Mobilisationsphase im Rollstuhl nicht rascher erreicht werden kann.
Abstract
Every year, about 1200 people in Germany are suddenly affected by para- or quadriplegia. In 25% of these cases occupational accidents are responsible, while in 25–30% the condition is precipitated by other underlying diseases. Modern spine surgery with minimally invasive techniques or with the risks of malpositioned pedicle screws kept to a minimum by application of the spinal neuronavigation system is one component of the overall treatment scheme currently applied in the case of such patients in SCI centers. The advantages of any early surgical stabilization achieved are lost if the patient concerned cannot be mobilized; immobilization involves the risk of pressure sores or urogenital tract infections, which can be prevented with early wheelchair mobilization. Complete repair of a separated spinal cord remains impossible. Treatment is therefore directed mainly at the complications of paralysis caused by the neurological lesions. Hypersecretion, thermal, vegetative and cardiac dysregulation – which can even take the form of cardiac arrest in the spinal shock phase – can be as much of a threat to life as the DVT and PE that occur with elevated frequency in such patients for up to 6 months after the accident.
