Zusammenfassung
Die Umstellungsosteotomie bei unikondylärer Gonarthrose dient einerseits der Verbesserung der Lebensqualität der Betroffenen, indem sie Schmerzen nimmt und die Gelenkfunktion verbessert, andererseits ist ihr ein präventiver Effekt zuzuschreiben, da sie durch die Verlagerung der Lastübertragung weg vom gonarthrotisch veränderten Kompartment hin zum gesunden die Progression der Arthrose verlangsamt. Diese Eingriffe sollten nur nach strenger Indikationsstellung und Ausschöpfung aller konservativen Therapieoptionen durchgeführt werden. Insgesamt erreichen sie ansprechende mittelfristige Ergebnisse, liegen aber langfristig nicht auf dem Niveau der Endoprothesen.
Abstract
Corrective osteotomy in the case of unicondylar gonarthrosis leads on the one hand to improved quality of life by attenuating the pain and improving the joint function; on the other hand it also has a preventive effect, in that weight-bearing is shifted from the knee compartment already affected by arthritis to the „healthy“ compartment, which slows down the progression of the ¶gonarthrosis. This kind of therapy is indicated only after having all modes of conservative treatment have been exhausted. The overall results of weight-transferring osteotomies are good in the medium term. The long-term results are not of quite the same quality as arthroplasty, however.
