Zusammenfassung
Frakturen des Ellenhakens kommen relativ häufig und in allen Lebensaltersstufen vor. Sie sind vielgestaltig und in der Regel nach Klinik und Röntgenbefund leicht zu diagnostizieren. Nur nichtdislozierte Brüche können konservativ, alle dislozierten Frakturen müssen operativ behandelt werden. Gewisse Besonderheiten in der Indikationsstellung sind im frühen Kindes- und im fortgeschrittenen Greisenalter zu beachten. Häufigst verwendetes Osteosyntheseverfahren ist die Zuggurtung, mit Drähten nach Weber oder in anderen technischen Varianten. Kombinationen mit Zugschrauben sind häufig erforderlich. Bei starker und irregulärer Zerstörung sind abstützende oder neutralisierende Plattenmontagen von Vorteil. Transfixationen mittels Fixateur externe kommen temporär bei Problemfrakturen und bei schwerem Weichteilschaden in Betracht. Bei der Nachbehandlung ist die erreichte Stabilität zu respektieren und funktionell zu nutzen. Unter korrekter Behandlung ist die Prognose der meisten dieser Verletzungen sehr gut bis gut. Standard in der Therapie ist das individualisierte Vorgehen nach bewährten Prinzipien!
Abstract
Fractures of the olecranon occur relatively frequently and at all ages. They are polymorphic and usually easy to diagnose from the clinical symptoms and X-ray images. Conservative treatment is indicated only for nondislocated fractures, all dislocated elbow fractures requiring a surgical operation. Particular care should be devoted to determining what treatment is indicated when the patients are in early childhood or in advanced old age. The most frequently used osteosynthesis is tension banding, either with wires or in some other technical variant. Combinations with compression screws are frequently necessary. In cases of severe and ¶irregular destruction buttress or neutralization plates are beneficial. Temporary transfixation with an external fixator should be considered for problem fractures and in the presence of severe soft tissue damage. During the aftertretment, the stability achieved must be respected and used functionally. With the right treatment, most of these lesions have a very good or a good prognosis. The standard therapy in olecranon fractures is therefore an individualized regimen according to proven principles.
