Die Zunahme antimikrobieller ¶Resistenzen bei Infektionserregern ist ein weltweites Problem unterschiedlichen Ausmaßes je nach Keim, Region oder auch Krankenhaus. In dieser Übersicht werden einige grundsätzliche Zusammenhänge zum Verständnis der Entwicklung antimikrobieller Resistenzen erläutert. Das Prinzip des Selektionsdrucks wird in seiner herausragenden Bedeutung für die Entwicklung und Verbreitung resistenter Erreger erläutert. Als Beispiel wird die Problematik der Oxacillin- und Multiresistenz bei Staphylokokken beschrieben. Strategien gegen die Zunahme antimikrobieller Resistenzen müssen eine koordinierte Politik auf überregionaler, regionaler und krankenhausinterner Ebene umfassen. Dies betrifft entsprechende Richtlinien und Gesetze sowie die Erfassung und Überwachung von Krankenhausinfektionen und mikrobiellen Resistenzentwicklungen ebenso wie eine rationale Antibiotikapolitik und effiziente Infektionskontrollmaßnahmen. Im Zentrum der krankenhaushygienischen Maßnahmen müssen die Erkennung einer Kolonisierung von Patienten oder auch Mitarbeitern mit multiresistenten Erregern (MRE) und die Verhinderung einer Übertragung und Verbreitung dieser Keime innerhalb des Krankenhauses stehen.
The growing resistance of microorganisms to antimicrobials is a global problem of varying degrees, depending on microorganism, region, and hospital. In this commentary, we discuss some basic principles for the understanding of antimicrobial resistance and clarify that selective pressure is the most important determinant in its development and spread. As an example, we describe the problem of methicillin and multi-drug resistance in staphylococci. Strategies for dealing with the problems of multi-drug-resistant microorganisms must include a coordinated policy based on national, regional, and hospital guidelines, surveillance of nosocomial infections and antimicrobial resistance, antibiotic regulations, and infections control. All these measures are necessary to meet the challenge of antimicrobial resistance. Here, the most important issue for hospitals is the detection of multi-drug-resistant microbes colonizing patients or staff and the control of their transmission and dissemination in the hospital.
