Perthes-Syndrom nach schwerer Thoraxkompression

Eine fast vergessene Diagnose?

The Perthes syndrome as a sign of severe thoracic compression. Centenary of its first description

Das Perthes-Syndrom, auch Perthes-Braun-Syndrom genannt, mit dem charakteristischen Bild der petechialen Blutungen und blauroten Zyanose von Hals und Gesicht sowie subkonjunktivalen Hämorrhagien gilt als wichtiges Zeichen einer schweren Thoraxkompression. Als ursächlich für diese Symptomatik wurde von Perthes eine akute Druckstauung der weitgehend klappenlosen venösen Gefäße der oberen Körperhälfte angegeben. Als Perthes-Druckstauung erhielt seine Theorie eine auch heute noch gültige internationale Anerkennung. In der neueren deutschsprachigen Literatur findet sie jedoch keine Erwähnung mehr – zu Unrecht, wie wir meinen. Wir berichten über 4 polytraumatisierte Patienten, bei denen wir die Symptomatik des Thoraxkompressionssyndroms beobachten konnten. Damit möchten wir an die inzwischen 100 Jahre alten, aber immer noch lesenswerten Perthes-Berichte von 1899 erinnern.

Cervicofacial cyanosis in combination with characteristic rash, petechial bleeding, and persistent subconjunctival haemorrhage were recognised as symptoms of severe thoracic pressure congestion by Perthes in 1899. He attributed it to acute compression in the predominantly valveless veins of the upper half of the body. This condition has since been internationally recognised as the so-called Perthes syndrome, but it seems to be forgotten nowadays, in our opinion wrongly: physicians may miss the diagnosis because of more urgent treatment of associated injuries or even fail to recognise its diagnostic importance. To remind of the centenary of its first description, we report on four polytraumatised patients with symptoms of Perthes syndrome from severe thoracic compression.
