Wirkprinzipien der winkelstabilen Platten-Schrauben-Verbindung bei Fixateur-interne-Osteosynthesen

Im Rahmen von Demonstrationsversuchen wurden typische Osteosynthesekonstruktionen unter Verwendung von Titanfixateur-interne-Implantaten an Kunststoffprobekörpern montiert und mechanisch belastet. Verglichen wurden eine herkömmliche mit einer ¶winkelstabilen Schraubenfixation (Fixateur interne) an der Platte. Bei Simulation einer Osteoporose oder limitierter Platten-Knochen-Kontaktfläche am Röhrenknochen und bei Simulation eines kurzen meta-/epiphysären Fragments mit frakturparallelen Schrauben erhöhte sich die Festigkeit deutlich durch ein winkelstabiles Schrauben-Platten-Interface. Bei einer Anordnung von 3 Schrauben in einem simulierten Femurkondylus wurden hohe Momente sowohl im Schrauben-Platten-Interface als auch durch die räumliche Abstützung der fixierten Schrauben im spongiösen Knochen übertragen.

For purposes of comparison, locked and unlocked osteosynthesis systems were mounted on plastic models and mechanically tested. The latter demonstrated considerably more stabilization with the simulations of osteoporosis, limited plate-to-bone contact in tubular bone, and for simulations of short meta- and epiphyseal fragments with screws inserted parallel to the fracture. Using three screws in a model of the femoral condyle, a high degree of rigidity was attained by both the angular stability of the screw fixation and the additional spacial anchoring of a rigid frame formed by the three screws in the cancellous bone tissue.
