In einer prospektiven Studie wurden die klinischen Verläufe von 82 polytraumatisierten Patienten analysiert, deren schwere Lungenschäden mit kontinuierlicher Rotationstherapie (kinetische Therapie) behandelt wurden. Indikation und Durchführung der Therapie erfolgten nach einem standardisierten Behandlungskonzept. Bei den Patienten handelte es sich um 67 Männer und 15 Frauen. Die kinetische Therapie wurde in 3 Patientengruppen durchgeführt. Gruppe I: ARDS (n = 25), Gruppe II: ALI (n = 26). Für das ARDS und ALI (acute lung injury) wurden die Kriterien der 1. und 2. ARDS-Konsensus-Konferenz zugrundegelegt. Gruppe III: Prophylaxe (n = 31). Als Prophylaxegruppe wurde ein Patientenkollektiv mit einer PaO 2 / FiO 2 -Ratio ≥ 300 evaluiert. Die Lagerungstherapie wurde zur Prophylaxe von Atelektasen bei entsprechend der Traumaschwere absehbarem längerem Intensivaufenthalt durchgeführt. Das Durchschnittsalter betrug (alle Werte angegeben als Mittelwert ± SD) für das Gesamtkollektiv 37,8 ± 16,38 Jahre. Der initiale ISS betrug 40,6 ± 18,4, der APACHE II am ersten Tag 13,3 ± 5,4. Die Liegedauer im kinetischen Bett betrug 6,95 ± 4,3, die Beatmungsdauer 19,7 ± 16,4 Tage. Die Patienten blieben 24 ± 17,9 Tage auf der Intensivstation und verließen nach 36,3 ± 29,5 Tagen die Klinik. 7,3% der Patienten des Kollektivs verstarben. Die konsequente kinetische Therapie integriert in ein standardisiertes Behandlungsregime trägt nach Ansicht der Autoren dazu bei, das bisher schlechte Outcome von schwerverletzten Patienten mit Lungenschäden zu verbessern.
The clinical progress of 82 patients with multiple trauma was analysed in a prospective study. Severe lung injuries in these patients were treated by means of continuous rotational therapy (kinetic therapy). Selection for this treatment and therapy itself followed a standardised treatment plan.The patients comprised 67 men and 15 women. Three treatment groups were studied. Group I consisted of patients with adult respiratory distress syndrome (ARDS) and group II, of patients with acute lung injury (ALI); patients in group III (prophylaxis) each had a P a O2/ FiO2 ratio ≥ 300 and were treated prophylactically. Positioning therapy was administered in group III to prevent atelectases during the relatively long period in intensive care expected in view of the severity of the trauma. For the total group the average age (all values are given as mean ± SD) was 37.8 ± 16.38 years. The initial ISS was 40.6 ± 18.4 and the APACHE II on the first day was 13.3 ± 5.4. The mean treatment time in the kinetic bed was 6.95 ± 4.3 days and the time on a respirator, 19.7 ± 16.4 days. The patients stayed on the ICU 24 ± 17.9 days and left the hospital after 36.3 ± 29.5 days. The mortality in the collective was 7.3%. Consistent kinetic therapy integrated in a standardized treatment regimen contributes towards improving the negative outcome to date of patients with severe lung injuries.
