Intraoperative Navigationssysteme

Die intraoperative Computerassistenz in Form einer Visualisierung der chirurgischen Aktion ist ein relativ neues Feld in der orthopädischen Chirurgie, das sich aus der Einbeziehung von präoperativen Daten aus bildgebenden Verfahren in den intraoperativen Ablauf entwickelt hat. Ziel ist es dabei, die aktuelle Lage und Ausrichtung von Instrumenten in Relation zur operierten Anatomie in Echtzeit an einem Monitor darzustellen, um dadurch Eingriffe genauer und sicherer durchführen zu können. Konzeptionell läßt sich jedes Navigationssystem in 3 Bereiche – Bild, Anatomie und ein sie verbindender Navigator – aufteilen. Die vorliegende Arbeit erklärt das grundsätzliche Funktionsprinzip eines solches Systems, beschreibt die heute in der Orthopädie und ihren verwandten Disziplinen bekannten intraoperativen Navigationssysteme und versucht, sie nach der Art des verwendeten Navigators zu klassifizieren.

Intraoperative computer assistance by means of image interactive visualization of the surgical action is still rather novel in orthopaedic surgery. It incorporates preoperative image data into the intraoperative procedure. The goal is to visualize the actual position and orientation of surgical instruments relative to the anatomical structure being operated on and display this information in real time on a computer monitor, to allow more precision and greater safety in performance of the intervention. In principle, each navigation system consists of three components: the image, the anatomical structure concerned, and a navigator linking the other two. This paper explains the functional basics of such a system and describes the navigation systems that are currently in use in orthopaedics and related disciplines. A classification based on the type of navigator used in each is also given.
