Zusammenfassung

Ein wesentliches Problem der Oberarmkopffrakturen ist die Einteilung. Es führt dazu, dass die Interobserverübereinstimmung in der Literatur nur zwischen 0,35 und 0,66 beträgt und führt auch zu Problemen in der Empfehlung spezieller Osteosyntheseverfahren für spezielle Frakturtypen. Aufgrund des häufig intakten Periostschlauchs gelingt die geschlossene Reposition und perkutane Osteosynthese mit Kirschner-Drähten oder Schrauben in einer überwiegenden Anzahl von 3- und 4-Part-Frakturen. Die T-Platten-Osteosynthese ist vor allen Dingen den ¶A-Frakturen vorbehalten, während bei schwerer Osteoporose, hohem Alter und ¶4-Part-Frakturen die prothetische Versorgung indiziert ist. Die besten Ergebnisse ¶lassen sich hier mit einer frühzeitigen Indikation zur prothetischen Versorgung erzielen.

Abstract

Classification is a serious problem with fractures of the head of the humerus. Because of this, interobserver agreement in the literature is only between 0.35 and 0.66. It also leads to problems when specific osteosynthesis techniques are to be recommended for specific types of fractures. Because the periosteum is frequently intact, closed reduction and percutaneous osteosynthesis with Kirschner wires or screws are successful in the majority of three- and four-part fractures. Osteosynthesis with T-plates is restricted mainly to A-class fractures, while insertion of a prosthesis is indicated in the presence of severe osteoporosis, in patients who have already reached an advanced age and in the case of four-part fractures. The best results are obtained when the advisability of a prosthesis is recognized at an early stage.
