Zusammenfassung
Schulterluxationen entstehen traumatisch oder auf der Grundlage konstitutioneller oder posttraumatischer Instabilitäten. Letztere werden mit Vorteil konservativ-funktionell behandelt. Traumatische Luxationen führen regelhaft zu strukturellen Schäden, die mit klinischen Funktionstests, Sonographie, MRT, Arthro-CT und Arthroskopie nachgewiesen oder ausgeschlossen werden. Bei jungen Patienten, etwa < 30 Jahren, aktiven Sportlern und Überkopfarbeitern liegt die Rezidivrate nach konservativer Therapie bei >85%. Hier ist daher die operative Therapie der traumatischen Schulterluxation dringend zu empfehlen. Alle anderen Patienten können dagegen konservativ behandelt werden, wenn grobe strukturelle Schäden ausgeschlossen wurden. Die Arthroskopie hilft, Binnenschäden zu identifizieren, und erlaubt, diese minimalinvasiv zu sanieren (Nähte, Anker, Stapler, Schrauben). Schon Gelenkspülung, Evakuation und Saugdrainage schaffen häufig die Voraussetzungen für gute Früh- und Langzeitergebnisse und sollten daher großzügig eingesetzt werden. ¶66 Schultergelenkerstluxationen des eigenen Krankenguts aus 20 Monaten wurden analysiert, und das Therapiespektrum wird vorgestellt.
Abstract
Shoulder dislocations are caused traumatically or by constitutional or posttraumatic instabilities. The latter are treated successfully with conservative-functional methods. Traumatic dislocations are mostly accompanied by structural lesions. Clinical examination, sonography, MRT, arthro-CT and arthroscopy can be used to detect or exclude these. In young patients, e.g. under 30 years of age, athletes and over-head workers the relapse rate is greater than 85%. In such patients operative treatment of traumatic shoulder dislocation is highly advisable. Other patients, in contrast, can be treated conservatively if serious structural lesions are excluded. Arthroscopy helps to identify structural lesions and makes minimal invasive treatment of them (with sutures, anchors, staplers, screws) possible. Even lavage, evacuation and suction drainage frequently result in the conditions needed for good early and long-term results to be achieved, and for this reason these procedures should be used generously. A series of 66 primary shoulder dislocations treated in 20 months is analysed and the range of therapies applied is presented.
