Verletzungen des Schultergelenks

Zusammenfassung

Die bildgebende Diagnostik des traumatisierten Schultergelenks hat in den vergangenen Jahren einen grundlegenden Wandel durchlebt. Nach erfolgter spezifischer Anamnese und spezieller klinischer Untersuchung stellt die konventionelle Röntgenuntersuchung in 2 Ebenen immer noch die Basis jeglicher weiterführender bildgebender Diagnostik dar. Mittels Sonographie, (Arthro)-Computertomographie, (Arthro)-MRT und Knochenszintigraphie können mehr spezifische Details bezüglich der Schwere und der Ausdehnung der Verletzungen, besonders der Rotatorenmanschettenmuskulatur und des kapsulolabralen Apparats, gewonnen werden. Um sinnlose Mehrfachuntersuchungen teurer Schnittbildverfahren zu vermeiden, werden die Möglichkeiten und Grenzen der einzelnen Methoden in Bezug zu den am häufigsten auftretenden Verletzungen aufgezeigt.

Abstract

The past few years have seen profound changes in diagnostic imaging of the injured shoulder. Once the history has been taken and a clinical examination conducted, standard radiographic evaluation (in two planes) is still the basis for any further diagnostic imaging. Ultrasonography, contrast computed (arthro-) tomography, (arthro-)MR and bone scintigraphy can be used to reveal more specific details of the extent and severity of shoulder injuries, particularly the status of the cuff musculature, the glenoid labrum and the capsule. To avoid unnecessary or repeated use of such expensive examinations, the potential usefulness and the limitations of each of these methods are shown with reference to the most common injuries.
