Zusammenfassung
Der Schlüssel zur erfolgreichen Behandlung der perilunären Luxationen und Luxationsfrakturen liegt in der sicheren Diagnosestellung, der Wiederherstellung der anatomischen Knochenstellung, stabilen Frakturversorgung, adäquaten Bandrekonstruktion und sicheren Transfixation der instabilen Gelenkanteile. Die konsequente Umsetzung dieser Prinzipien erfordert eine frühzeitige und offene operative Versorgung sowie eine langwierige Nachbehandlung. Da es sich um seltene Verletzungen mit einem komplexen und sehr variablen Verletzungsmuster handelt und die Behandlung sowohl gute Kenntnisse der biomechanischen Besonderheiten an der Handwurzel als auch spezifische operationstechnische Erfahrungen erfordert, sollten diese Patienten möglichst in handchirurgischen Zentren mit größeren Fallzahlen behandelt werden. Den Betroffenen kann unter diesen Voraussetzungen in den meisten Fällen eine gute Wiederherstellung der Handgelenkfunktion in Aussicht gestellt werden, wobei sich aufgrund von traumatisch bedingten Knorpelschäden, Knochennekrosen und chronischen Bandinstabilitäten langfristig bei nicht wenigen Patienten posttraumatische Handgelenkarthrosen entwickeln. Perilunäre Verletzungen erleiden in der Regel jüngere, erwerbstätige und sportlich aktive Menschen, sodass nicht selten berufliche und soziale Probleme bis hin zur Notwendigkeit des Berufswechsels resultieren und damit auch erhebliche Kosten für die Versicherungsträger.
Abstract
Correct diagnosis, anatomical reduction, stable fracture fixation, adequate ligament reconstruction and a secure transfixation of unstable joints are the key to successful treatment of acute perilunate dislocations and luxation fractures. Realization of these principles necessitates an early open surgical procedure and a protracted follow-up treatment. These infrequent injuries should be addressed at a specialized center for hand surgery due to the variable and complex injury patterns which require a precise biomechanical knowledge of the carpus and specific surgical experience. By following these prerequisites a good restoration of wrist function is expected to be achieved in most cases. In a number of patients, chondral lesions, avascular osteonecrosis and carpal ligament instability lead to posttraumatic osteoarthritis requiring a secondary salvage procedure. Perilunate injuries are a domain of younger working and active men and are often related to social and occupational problems with returning to work sometimes requiring occupational reorientation, which can result in substantial costs for health insurance companies.
