Hintergrund
Die posttraumatische Arthrose ist eine häufige Komplikation nach Frakturen im Bereich des Sprunggelenks und des Rückfußes.
Methoden
Die konservative Therapie mit dem Ziel der Schmerzreduktion ist zunächst Mittel der Wahl. Durch eine adäquate Belastung und Trainingstherapie, kombiniert mit orthopädischen Hilfsmitteln und oralen Analgetika, kann eine operative Therapie hinausgezögert werden. Erst nach Ausschöpfen dieser Maßnahmen ist bei nichttolerierbarer Schmerzsymptomatik oder instabilem Gelenk die Versteifung des Gelenks in Erwägung zu ziehen. Die Arthrodese eines oder mehrerer Gelenke verfolgt das Ziel der Schmerzreduktion und der Stabilitätssicherung.
Ergebnisse
Durch eine Vielzahl an beschriebenen Operationstechniken, die orientierend in externe oder interne Verfahren unterteilt werden, ist eine hohe Konsolidierungsrate mit hoher Patientenzufriedenheit zu erreichen. Trotz durchgeführter Arthrodese kann der Patient suffizient in den Alltag und das Berufsleben eingegliedert werden. Somit ist die Arthrodese des Sprunggelenks und Rückfußes als ein sicheres und bewährtes Verfahren anzusehen.
Abstract
Background
Posttraumatic osteoarthritis is a common long-term complication following fractures of the ankle and hindfoot.
Methods
The initial therapy is a non-operative conservative treatment, which consists of physiotherapy and oral analgesics. A sufficient conservative treatment is capable of delaying a surgical procedure. Arthrodesis of the ankle joint and hindfoot is an effective treatment for addressing pain in patients with osteoarthritis. The conservative treatment options should be completely exhausted before any surgical interventions are carried out.
Results
In the literature numerous operation techniques are described which can be differentiated into internal or external fixation procedures. Both forms of technique have a high rate of fusion and are accompanied by a good clinical outcome. The main objectives following arthrodesis are complete reintegration in the former profession and daily life activities.
