Hintergrund
Ossifikationen sind gutartige Knochenneubildungen unterschiedlichster Genese. Bei akuter Querschnittlähmung stellen neurogene heterotope Ossifikationen eine relativ häufige Komplikation dar. Sie können die Beweglichkeit der betroffenen Gelenke erheblich einschränken bis unmöglich machen.
Diagnose
Insbesondere die frühe Diagnostik ist aufgrund einer unklaren Symptomatik und fehlender klarer Laborkonstellationen erschwert. Im Falle einer akuten Querschnittlähmung sollte daher in regelmäßigen Abständen eine standardisierte Ultraschalluntersuchung durchgeführt werden, mit Beginn möglichst in den ersten 3 Wochen nach Eintritt der Lähmung, bei klinischen Symptomen sofort. Beim Auftreten pathologischer Schallmuster wird die Diagnose magnetresonanztomographisch gesichert und das Bestrahlungsfeld wird festgelegt.
Therapie
Die betroffenen Areale sollten nach Bestätigung der Pathologie durch das MRT möglichst ohne Verzögerung mit einer Dosis von insgesamt 5-mal 3 Gy fraktioniert bestrahlt und anschließend regelmäßig nachuntersucht werden.
Schlussfolgerung
Mit konsequent durchgeführten Früherkennungsmaßnahmen können laut eigenen Daten über 95 % aller heterotopen Ossifikationen im Frühstadium erkannt und erfolgreich behandelt werden.
Abstract
Background
Ossification is a benign new formation of bone due to a wide variety of reasons. In acute paraplegia neurogenic heterotopic ossification is a relatively common complication which can substantially limit mobility of the affected joint and even make movement impossible.
Diagnosis
Early diagnosis is particularly difficult due to unclear symptoms and the lack of a clear set of laboratory parameters. In cases of acute paraplegia a standardized ultrasound examination should be carried out at regular intervals beginning if possible in the first 3 weeks after the first appearance of paralysis and if clinical symptoms are present should begin immediately. In the presence of pathological ultrasound patterns the diagnosis is confirmed by magnetic resonance imaging (MRI) and the area to be irradiated is determined.
Therapy
After confirming the pathology by MRI the affected area should be irradiated by a total of 5 times 3 Gy in a fractionated process with as little delay as possible and follow-up examinations should be regularly carried out.
Conclusions
If early diagnostic measures are consistently carried out the data show that 95 % of the cases of heterotopic ossification can be recognized at an early stage and successfully treated.
