Zusammenfassung
Aktuell gibt es wenig valide Informationen über die Behandlung implantatassoziierter und periprothetischer Frakturen am Humerus. Deren Inzidenz ist niedrig, jedoch ist künftig mit einer Zunahme zu rechnen. Die Behandlung ist aufgrund der schlechten Knochenqualität der häufig älteren Patienten problematisch. Die konservative Therapie resultiert oft in verzögerter Heilung oder Pseudarthrosen mit Ermüdungsversagen der Implantate. Therapieziele sind die sichere Implantatfixierung, eine ausreichende Stabilität zur knöchernen Konsolidierung und die stabile Protheseneinheilung, um eine ausreichende, schmerzfreie Bewegung zu ermöglichen. Die Revision mit einem die Fraktur übergreifenden Schaft ist v. a. bei gelockerten Prothesen und bei schlechter Knochenqualität indiziert, bei guter Knochenqualität kann eine überbrückende Osteosynthese mit winkelstabilen Platten und Drahtcerclagen zur Anwendung kommen. Bei implantatassoziierten Frakturen am Übergang vom versteiften zum physiologisch flexiblen Knochen können lange, den Defekt ausreichend überbrückende Plattensysteme verwendet werden. Mit neuen intramedullären Implantaten lässt sich sowohl bei primärer Osteosynthese als auch bei Revisionen eine übungsstabile Situation am Humerus erreichen.
Abstract
Only scant information is currently available on the treatment of implant-related and periprosthetic fractures of the humerus. Although the incidence of these fractures is low, an increase is expected in the future. Treatment is challenging since patients are often elderly and bone quality is poor. Conservative treatment often leads to delayed healing and non-union. Treatment goals include reliable implant fixation, sufficient stability to enable bony healing and stable osseointegration of the prosthesis for pain-free and useful joint function. Whereas long-stem revision surgery is performed in cases of loose stems and poor bone quality, overlapping osteosynthesis with angle stable plating systems and cerclages can be used in cases of good bone quality. With implant-related fractures at the junction between rigid bone due to an implant and flexible bone, revision surgery using long angle-stable plates or overlapping plates additional to the remaining implants can be accomplished. New intramedullary implants (long proximal humerus nails) can be successfully used either in primary osteosynthesis or in cases of revision and can achieve sufficient stability for early functional aftercare.
