Zusammenfassung
Die künftige Struktur der stationären Heilverfahren in der Deutschen Gesetzlichen Unfallversicherung (DGUV) sieht ein an die Systematik des TraumaNetzwerkD DGU (DGU: Deutsche Gesellschaft für Unfallchirurgie) angepasstes 3-stufiges System vor: unfallchirurgische Basisversorgung, Versorgung von Verletzungen des VAV-Katalogs (VAV: Verletzungsartenverfahren) sowie von Fällen mit besonders hohem Versorgungsaufwand. Neben der Akutversorgung soll auch die Rehabilitation eine bedarfsgerechte Zuweisungssteuerung erfahren, wobei der Verzahnung der Versorgungssektoren eine wichtige Rolle zukommt. Durch Einführung eines Qualitätssicherungssystems, Bedarfsorientierung und vorgeschriebene Weiterbildungsmaßnahmen soll die Qualität der Versorgung optimiert werden. Die Forderungen der DGUV werden vom Verband der für die BG tätigen Ärzte als grundsätzlich sinnvoll und nachvollziehbar angesehen, müssen jedoch eine entsprechende Stärkung der Position der Durchgangsärzte am Krankenhaus nach sich ziehen. Das größte Konfliktpotenzial zwischen DGUV und Ärzten liegt nicht in der Nachvollziehbarkeit der erhobenen Forderungen nach Qualitätssicherung und flächendeckender Versorgung, sondern in deren Umsetzbarkeit.
Abstract
The future structure of in-patient medical treatment offered by the German statutory accident insurance (DGUV) makes provision for a three-staged system adapted to the systematization of the German DGU trauma network (DGU, German Society of Trauma Surgery): basic trauma surgical treatment, treatment of injuries according to German injury type procedures, as well as cases requiring particularly intensive treatment. In addition to acute care, rehabilitation should also be apportioned appropriate importance, whereby the interlocking of care sectors is of particular importance. Optimizing the quality of care is guaranteed by the introduction of a quality control system, needs orientation and defined advanced training measures. DGUV requirements are considered useful and important by physicians active on behalf of employers’ liability insurance associations, although the position of medical examiners in hospitals requires strengthening. The greatest potential for conflict between the DGUV and physicians lies not in the plausibility of the requirements for quality assurance and nationwide care, but rather in their practicability.
