Zusammenfassung
Infolge der prekären Weichteilsituation am distalen Unterschenkel und Rückfuß kommt es dort häufiger zu Infektkomplikationen, die sich früh manifestieren und schwierig zu behandeln sind. Beim Vorliegen von Risikofaktoren kann die Gliedmaße bedroht sein. Zur Behandlung von tiefen Infektionen ist deshalb ein strukturierter Algorithmus erforderlich. Eine Literaturrecherche ergab 5 relevante, im vorliegenden Beitrag berücksichtige Studien, evidenzbasierte Empfehlungen wurden bisher nicht publiziert. Liegt bei der Frühinfektion das Metall regelrecht ein, ist ein Erhaltungsversuch indiziert. Ist die Osteosynthese gelockert, sind meist eine vollständige Metallentfernung und ein Verfahrenswechsel angezeigt. Beim Spätinfekt (ab der 5. postoperativen Woche symptomatisch) ist das Implantat in vielen Fällen mit einem Biofilm besetzt und muss deshalb entfernt werden, das Lager ist sorgfältig zu debridieren. Ist die Fraktur konsolidiert, sind keine weiteren Maßnahmen erforderlich, andernfalls sind Verfahrenswechsel und Reosteosynthesen notwendig. Weichgewebedefekte können in dieser Region sehr regelhaft und mit guter Erfolgsrate mit einem oder mehreren gestielten fasziokutanen Lappen verschlossen werden.
Abstract
Posttraumatic infectious complications of the distal lower leg and hindfoot develop more often and are difficult to treat compared to other anatomical regions, mainly due to the thin and vulnerable soft tissue envelope. If the patient bears risk factors, limb-threatening conditions might result. It is therefore necessary to follow a structured therapy algorithm. A literature search identified five relevant studies, which have been considered in the present article; evidence-based results have not been published as yet. Dislocated fixation devices must be removed and a conversion procedure is required. As for late infections, bacterial biofilm on the implant demands complete removal and resection of dead bone. If the fracture is consolidated, further procedures are rarely necessary. In cases of an infected nonunion, reconstruction and multiple revisions may be necessary. Soft tissue defects in that region can mostly be covered with one or more pedicled fasciocutaneous regional flaps.
