Zusammenfassung
Schädel-Hirn-verletzte Patienten bedürfen in allen Phasen der Traumabehandlung neurochirurgischer Versorgung. In der Primärphase werden raumfordernde intrakraniale Hämatome, penetrierende Verletzungen, Impressionsfrakturen und Weichteilverletzungen versorgt. In der Sekundärphase (intensivmedizinische Phase) werden, soweit nicht bereits primär erfolgt, intrakraniale Messsonden (Hirndruck, Gewebesauerstoff) mit oder ohne Liquorableitung implantiert. Bei konservativ nicht beherrschbarem Hirndruck oder progredienten intrazerebralen Hämatomen (Kontusionsblutungen) kommen dekompressive Kraniektomien mit Duraerweiterungsplastiken zur Anwendung. Chirurgische Maßnahmen in der Tertiärphase haben meist rekonstruktiven Charakter. Rhinoliquorrhöen mit Frakturen der Frontobasis werden über den intraduralen subfrontalen Zugang abgedeckt, in Ausnahmefällen auch extradural oder transnasal-transsphenoidal. Schädeldachdefekte werden durch frei oder unter Einsatz von CAD („computer-aided design“) geformte Plastiken versorgt. Bevorzugte Materialien sind Kunststoffe, Keramiken oder Titan. Schließlich können in der Tertiärphase auch Hirnwasser ableitende Eingriffe erforderlich werden, vorzugsweise ventrikuloperitoneale Ableitungen.
Abstract
Patients with traumatic brain injury need neurosurgical care throughout the treatment process. The acute phase is dominated by evacuation of intracranial hematomas and the treatment of penetrating injuries, depressed skull fractures and soft tissue damage. Neurosurgical procedures of the second (intensive care unit) phase are implantation of intracerebral sensors for brain pressure and tissue oxygen (ptO2) and decompressive craniectomy to treat intracranial pressure and progressive intracerebral hematomas (contusions). Neurosurgical procedures of the third phase are mostly of a reconstructive nature. Frontobasal injuries with cerebrospinal fluid (CSF) leakage necessitate subfrontal intradural reconstruction and coverage of the skull base, skull defects need closure by freehand or CAD-constructed implants and hydrocephalus requires implantation of ventriculo-peritoneal shunts.
