UV-induzierter berufsbedingter Hautkrebs

Zusammenfassung

Das invasive Plattenepithelkarzinom der Haut als Wie-Berufskrankheit nach § 9 Absatz 2 SGB (Sozialgesetzbuch) VII entsteht typischerweise auf chronisch lichtgeschädigter Haut aus aktinischen Keratosen (Carcinomata in situ). Für diese noch ganz oberflächlichen, intraepithelialen Karzinome existieren exzellente Therapie- und Präventionsmöglichkeiten, sodass bei frühzeitiger Diagnostik und Einleitung von § 3-Maßnahmen das Auftreten invasiver Karzinome mit ihren möglichen Konsequenzen (eingreifende, evtl. entstellende Therapiemaßnahmen, Metastasierung, letaler Verlauf) mit hoher Wahrscheinlichkeit vermieden werden kann. Zur Früherfassung dieser Fälle ist ein dermatologisches Hautkrebsscreening und zur Prävention ein frühpräventives Verfahren analog zum Hautarztverfahren zu fordern.

Abstract

Invasive squamous cell carcinoma of the skin considered as a type of occupational disease according to § 9 paragraph 2 of the Social Code VII typically develops on chronically sun-damaged skin from actinic keratoses (carcinomata in situ). There are excellent treatment and preventive options available for these intraepithelial carcinomata that are still superficial so that when they are diagnosed early and measures according to § 3 are instituted promptly it is highly probable that the occurrence of invasive carcinomata with their possible consequences (invasive, potentially disfiguring treatment, metastatic disease, lethal outcome) can be avoided. For early detection of these cases dermatological skin cancer screening should be required and for prevention early preventive measures should be provided analogous to the dermatologist’s procedure (Hautarztverfahren) in the prevention of occupational dermatitis.
