Zusammenfassung
Defektwunden stellen eine besondere technische und zeitliche Herausforderung an den Chirurgen dar. Um den Untergang von freiliegenden Strukturen zu vermeiden, ist eine schnelle Defektdeckung erforderlich. Voraussetzung hierfür ist ein ausgiebiges und radikales Débridement von verletzten oder infizierten Strukturen mit anschließender Defektdeckung innerhalb von 2–3 Tagen. Langwierige Okklusionsverbände hingegen können zu weiteren Nekrosen von tiefer liegenden Strukturen führen und sollten nur eine kurzzeitige, überbrückende Maßnahme darstellen. Zur Defektdeckung steht eine Vielzahl von lokalen und freien Lappenplastiken zur Verfügung. Die Auswahl der jeweiligen Lappenplastik richtet sich zum einen nach der Defektgröße und zum anderen nach den jeweiligen Vor- und Nachteilen, die mit den Patientenwünschen abgeglichen werden müssen. Nicht zuletzt entscheiden auch die Erfahrung, Vorliebe und Ausbildung des behandelnden Chirurgen über die Art der Versorgung.
Abstract
Substance defects are particularly challenging for surgeons in terms of both technique and time. To avoid the destruction of underlying structures, prompt defect coverage is required. For this, extensive radical débridement of injured or infected structures is necessary, followed by defect reconstruction within 2–3 days. Occlusion dressings worn for a protracted period of time, in contrast, can lead to further necrosis from deeper structures and should only be used as a temporary measure. There are a number of local and free reconstructive flap procedures available for defects. Choosing the correct procedure depends on the size of the defect on the one hand, and on the advantages and disadvantages of the various procedures on the other. Moreover, the latter need to matched against the wishes of the patient. Finally, the level of experience, preference and training of the treating surgeon are decisive factors in the choice of treatment.
